Is It Time to Update Your Beneficiary Forms?

Take this short quiz to determine if it’s time to create or update your beneficiary forms.

  • It has been three or more years since you last reviewed your forms.
  • You recently retired or changed jobs.
  • Your marital status has changed.
  • Your health status has changed.
  • You welcomed a new child or grandchild.
  • You would like to leave a gift to charity through a beneficiary designation.
Checking just one means it’s time to create or update your forms.

  • It costs you nothing now. You retain full control of your assets for as long as you need them.
  • There is no need to go to the trouble or expense of creating or updating a will — you simply fill out a form.
  • You are not locked in to a decision you make today — you can change your beneficiaries at any time.
  • You can designate your gift for the City of Hope program of your choice.
  • There is no minimum donation.
  • You can make City of Hope a contingent beneficiary, which puts us next in line to receive the funds, after family.
  • You can designate City of Hope to receive a percentage of the account value, leaving the remainder to your heirs.
  • Bank, Certificates of Deposit and Brokerage Accounts: Ask your bank for this easy-to-complete form.
  • Life Insurance Policies: The owner of a life insurance policy has a great deal of flexibility in naming beneficiaries and can generally name anyone he or she chooses.
  • Donor Advised Funds: The final distribution of contributions remaining in a Donor Advised Fund is governed by the forms you completed when you created your fund. These funds may be left to charity (or your heirs can continue to make periodic distributions to City of Hope and other charities from your account, if they are named as successor advisors).
  • Retirement Accounts: With an IRA, you can name any beneficiaries you want, including friends, family members, a trust or a charity. For a 401K or other workplace plan, you will need written permission from your spouse to leave it to anyone other than your spouse.

Like many other members of the City of Hope family, Sarah and Sam Richardson found how easy it is to take a small step that can help City of Hope patients and their families for years to come. “We were taught that you give what you can and we want to send that same message to our children. That’s why we’ve left a gift of life insurance to City of Hope. It’s an easy way to provide for our daughters and leave a legacy that gives hope to future families.”
